Why this pathway exists
Continuity is a governed life-course problem. Useful knowledge must survive handover while authority, privacy, identity, and current truth can mature or be revised.
Support often resets at the exact moments continuity matters most: a new carer, setting, service threshold, adulthood, provider failure, or a parent’s death.
Phase One establishes a research position. It does not claim a deployed care system, clinical authority, or a finished product.
